This is pretty shocking. It’s not just that the air traffic controller may have been asleep or inattentive. It’s that there was only one on duty in the first place at a busy airport in such a large urban area.

I keep asking the question—and this article asks it, too. Unfortunately, the answer is: we haven’t a clue. [ADDENDUM: Brian Fairchild says that we do know the answer, and it’s not a pretty one.]
